Alert and Notification System Activation Request

Utilize this form to request an Alert and Notification System be activated for your jurisdiction or incident.  

  • These systems include: the Emergency Alert System, Sirens, Wireless Emergency Alerts, AlertSLO, or Reverse 911 (managed by Sheriff - but OES can coordinate).  
  • If you are unsure which system is appropriate, consult with OES Duty Officer at (805) 458-0138.
  • This form should be used for new protective actions as well as cancellations/all clear or repopulations.
  • Information will also be posted on ReadySLO.org.  
  • Please confirm receipt with the Duty Officer at 805-458-0138 once form is submitted.
